The BMS service report, done in the plant room and sent as a branded PDF
A BMS service report records what you checked on a building management system visit, what you found and what you put right. Maintivo's ready-made BMS: Service & Inspection form takes you through panels, sensor readings, control, alarms and backups one tap at a time, then turns the visit into a BMS service report with your logo, ready to send.

- A ready-made BMS: Service & Inspection form with eight pages, for routine quarterly, seasonal and annual visits.
- Each check is one tap: Pass, Fail or N/A. A Fail asks for the defect, the fix and the priority.
- Sample sensor readings sit beside a reference instrument and are judged within ±1 °C, ±5 %RH or ±100 ppm.
- The annual checks only appear on an annual visit.
- The PDF carries your logo. Copy the form into your own template if a contract needs something different.
- Maintivo Pro is £34.99 a month, no contract, and your first report is free.
What is a BMS service report?
It is the record of a routine visit to a building management system: the state you found it in, the checks you made on the panels, network, sensors, control, alarms and backups, anything that failed and what you did about it. The client, the block manager or the managing agent keeps it as proof the system was looked after.
In Maintivo you fill it in on your phone as you go round the plant, and the report is finished when the visit is. Nothing is typed up later from a notebook.
What does the BMS service form check?
The form covers the checks a routine BMS service visit usually includes, in the order you would do them. It is a building management system service checklist that ends in a signed PDF.
| Page | What the engineer records |
|---|---|
| 1. Visit | Visit type (Quarterly, Seasonal or Annual), the BMS make (Trend, Honeywell, Siemens, Schneider Electric, Niagara / Tridium or Other), the plant or areas covered, the supervisor or software version, and the state the system was found in. |
| 2. Panels and network | Panels clean, dry and secure; wiring and terminations; controller status lights; power supplies and backup batteries; controllers online (the number expected against the number online, which must match); network comms; controller clocks. |
| 3. Sensors and outputs | The reference instrument and its calibration date; sample readings of outside air, space, heating flow and chilled water flow temperature, humidity and CO₂, each judged within tolerance; other sensors checked; valves and actuators respond when commanded. |
| 4. Control | Setpoints match the agreed values; time schedules are correct; hand overrides: none found, cleared, or left in hand with the reason. |
| 5. Alarms and logs | A test alarm reached the right person; repeat or nuisance alarms; key trend logs reviewed with no gaps. |
| 6. Backup and access | Backup taken, its date and where it is stored; user accounts reviewed. The form says never to enter passwords. |
| 7. Annual checks | Shown only on an annual visit: graphics spot check; firmware and software up to date; backup restore tested; safe state on comms loss; restarts after power loss; fire signal response (an observation only, not a fire system test). |
| 8. Finish | Work carried out, energy savings spotted, and the system left in automatic. |
How are sensor readings judged within tolerance?
You note the reference instrument and its calibration date first. Then, for each sampled sensor, you enter what your instrument reads and what the BMS shows. Maintivo works out the difference in real units and marks each one within tolerance or not. The readings and the PDF show the result for every sensor, so the client can see at a glance which ones need attention.
| Sensor sampled | What you enter | Within tolerance when |
|---|---|---|
| Outside air temperature | Reference reading and BMS reading | Within ±1 °C |
| Space temperature | Reference reading and BMS reading | Within ±1 °C |
| Heating flow temperature | Reference reading and BMS reading | Within ±1 °C |
| Chilled water flow temperature | Reference reading and BMS reading | Within ±1 °C |
| Humidity | Reference reading and BMS reading | Within ±5 %RH |
| CO₂ | Reference reading and BMS reading | Within ±100 ppm |
What happens when a check fails?
Every check is one tap: Pass, Fail or N/A. When you tap Fail, the form asks for three things: the defect, the fix and the priority. So a failed check never goes out as a bare cross. The client gets what is wrong, what was done or needs doing, and how soon.
Is it a commissioning record?
No. The form is built for a routine service visit to a system that is already running. It samples sensors rather than proving every point. For handover paperwork on other plant, Maintivo has separate forms, such as HIU commissioning and fan coil and MVHR records.
Can I change the form to match my contract?
Yes. Any form in Maintivo can be copied into your company's own template and changed. If a contract asks for extra plant, a different order or your own wording, copy the BMS form, make it yours and the whole team uses that version. More on this in M&E contractor software.
The BMS paperwork problems it fixes
BeforeThe service sheet is a Word file, and every engineer fills it in differently.
With MaintivoOne ready-made form, with the same checks in the same order on every visit.
BeforeSensor readings go in a notebook and get compared later, if at all.
With MaintivoReference and BMS readings side by side, each marked within tolerance or not.
BeforeA failed check goes out with no detail of what is wrong.
With MaintivoEvery Fail asks for the defect, the fix and the priority.
BeforePlant is left in hand and nobody knows why.
With MaintivoHand overrides are recorded: none found, cleared, or left in hand with the reason.

See a finished report before you download anything
One of the reports Maintivo produces (this one is an F-Gas leak check). A BMS service report carries the same things: your letterhead, the client and site, the checks and readings, and both signatures.
On your iPhone on site. On Android, a tablet or the office computer too.
One account for the whole firm. A report an engineer finishes on site is on the office screen as soon as it uploads, and a customer added at the desk is on every phone.
- iPhone: the app. It works with no signal and uploads when you are back in range.
- Android phones, iPads and other tablets: the web app at app.maintivo.com. Add it to the home screen and it opens like an app.
- Laptop or desktop: the same web app on a big screen, for reviewing reports, clients, invoices and the team.
- Always in step: the same reports, customers and PDFs everywhere. One subscription covers all of it.

A BMS engineer app for the firm and the client
BMS and controls engineers
Tap through the visit in the plant room. Readings, failed checks and overrides are captured as you go, and the report is done when you are.
M&E contractors
Every engineer follows the same checks, so every BMS report that leaves the firm looks the same, with your logo on it. Engineers on their own phones see their own jobs only.
Block managers and managing agents
A clear PDF for each visit: what was checked, which sensors were within tolerance, what failed, the fix and the priority. Each report is kept against the site, so the maintenance history for each property builds up by itself.
Where do the BMS reports go after the visit?
Every report is saved against the client and the site address. Every site address on a report goes into your list of sites automatically, and each site shows every report at that address, newest first. So when a managing agent asks what was found on the last quarterly visit, you open the site and it is there. See maintenance history by property for how that works across a block.
Does it work in a plant room with no signal?
In the iPhone app, yes. The visit, photos and signatures save on the phone and upload by themselves when you are back in range. On Android, a tablet or a computer you use the web app at app.maintivo.com, which needs a connection and keeps a draft in the browser.
What else can the same account do?
The same account covers the rest of a building services visit: HVAC and air conditioning service reports with the F-Gas sums done for you, general work inspections and customer sign-off on site. See every feature.
How much does it cost?
Your first report is free. After that, Maintivo Pro is £34.99 a month for the owner account, with a free trial: 3 days in the iPhone app, 14 days on the web. Each engineer you add needs a seat: £8.99 for 1, £24.99 for 3 or £34.99 for 5, a month. Invoicing is an optional £9.99 a month. Monthly, no contract. See full pricing.
Maintivo helps you record the visit properly. You carry out the checks, you sign the record and you remain the competent person.

Maintivo does not claim the form meets any particular standard. It covers the checks a routine BMS service visit usually includes, and you can copy it into your own template to match the schedule your contract uses. You carry out the checks, you sign the record and you remain the competent person.
No. The backup and access page asks whether user accounts were reviewed, and the form says never to enter passwords. The report records that the review was done, not the log-ins.
